Each location features a collaborative Care Team, including physicians, care coaches, behavioral health specialists, clinical pharmacists, and other professionals who provide holistic, 24/7 support.

Responsibilities

  • Provide patient care in alignment with established medical standards.
  • Ensure high-quality care by reviewing patient data and medical charts.
  • Coordinate patient services, including specialist referrals, hospital and skilled nursing facility (SNF) transitions, durable medical equipment, and home health care.
  • Actively collaborate with the care team in daily huddles, offering medical expertise and guidance.
  • Support the Regional Medical Director and Center Administrator by fostering a cooperative and professional practice environment.
  • Complete medical documentation promptly, working with a quality-based coder to enhance coding accuracy.
  • Adhere to clinical policies and protocols as outlined by leadership.
  • Meet regularly with the Regional Medical Director to review quality of care, outcomes, policies, procedures, and records management.
  • Contribute to the expansion of new or existing services within the center.
  • Participate in the local primary care “on-call” program as needed.
  • Maintain compliance with all licensing, certification, and accreditation requirements.
  • Devote 100% of work time to clinical patient care, including direct interactions and related administrative responsibilities such as charting and meetings.

Required Qualifications

  • Medical degree (MD or DO) from an accredited institution.
  • Board Certification (or eligibility) in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA).
  • Current, unrestricted medical license or willingness to obtain one in the practicing state; ability to obtain additional licenses as needed.
  • Strong verbal and written communication skills.
  • Exceptional interpersonal skills to effectively engage with patients and colleagues.
