Responsibilities:
- Educate and advise consumers and staff on medical health, medications, and related health topics.
- Provide personalized, one-on-one training to address specific individual needs.
- Design and lead group sessions and training programs for consumers and staff, covering key health and wellness areas such as nutrition, physical activity, and smoking cessation.
- Assist consumers in accessing community resources that support their health and wellness goals.
- Offer guidance and support to help consumers better understand their medical conditions.
- Meet with consumers and staff to discuss medical diagnoses, causes, symptoms, potential complications, and available treatment options.
- Deliver direct medical care as appropriate and as prescribed by a physician, including monitoring vital signs, wound care, and other interventions aligned with the consumer’s Individualized Action Plan (IAP) for health and wellness goals.
- Review medical documentation for each consumer at respite or as assigned, providing medical consultation and coordinating care as necessary.
