Position Overview:
We are looking for experienced Registered Nurses (RNs) to join our telehealth call center team. In this role, you will be responsible for triaging patient concerns, providing nursing guidance, and coordinating care remotely. The ideal candidates will have strong clinical expertise, exceptional communication skills, and the ability to excel in a high-volume, fast-paced environment.

Key Responsibilities:

Conduct telephonic triage, addressing patient health concerns and offering appropriate nursing guidance.

Assess patient conditions and recommend care based on clinical judgment and established protocols.

Identify potential care gaps and provide ER diversion guidance when necessary.

Accurately document patient interactions using NextGen, EPIC, and other EMR systems.

Coordinate services and referrals to healthcare programs and community resources.

Work collaboratively with physicians, case managers, and pharmacists to ensure seamless patient care.

Educate patients on treatment options, preventive care, and health management strategies.

Achieve key performance indicators (KPIs), including handling 50+ calls per day (for LPNs in RX).
Required Qualifications:

Active, unrestricted RN license in New York (NY).

Minimum 2 years of clinical experience (including pediatrics or OB/GYN for relevant roles).

At least 1 year of experience with EPIC EMR.

Background in triage and care coordination.

Strong technical proficiency, including Microsoft Office and EMR systems.

Ability to work independently in a high-volume, fast-paced environment.

Weekend availability is mandatory.

Home office setup with high-speed internet and a distraction-free workspace.
Preferred Qualifications:

1+ year of telehealth or phone triage experience.

Familiarity with healthcare protocols and procedures in Adult Medicine, Pediatrics, or OB/GYN.

Strong documentation skills and ability to adhere to structured processes.
