Responsibilities
- Collaborate closely with case managers to develop personalized care plans.
- Coordinate care with various providers, including primary care, specialists, mental health professionals, and home health services.
- Conduct in-home or community visits to assess client needs.
- Perform skin assessments for pressure injuries and wounds, providing recommendations for appropriate care.
- Educate and train caregivers, clients, and staff on best practices in care.
- Facilitate referrals to services such as Adult Day Health programs and medication management.
- Utilize Care Transitions strategies to prevent unnecessary hospital readmissions.
- Document assessments, interventions, and findings in the CARE system using an assigned laptop.
- Apply critical thinking skills to identify gaps in care plans and assist case managers in addressing them.
- May manage a small caseload of clinically complex clients.
- Advocate for clients, identifying and removing barriers to care.
Qualifications
Minimum Requirements:
- Education: Bachelor of Science in Nursing (BSN).
- Experience:
- At least 2 years of clinical nursing experience working with elderly and/or disabled adults, OR
- At least 3 years of clinical nursing experience with any age group.
- Alternative Qualifications: A combination of education, training, and/or work experience that demonstrates the ability to perform the responsibilities of the role.
Licenses & Certifications
- Active Washington State Registered Nurse (RN) license in good standing.
- Current Basic Life Support (BLS) certification.
- Valid Washington State driver’s license or equivalent mobility, with reliable personal transportation.
- Ability to lift, pull, or push at least 25 pounds.
- Up-to-date vaccination or occupational health records, or proof of immunity.
