Home Health Physical Therapist – Role & Responsibilities

As a Home Health Physical Therapist, you will:

  • Plan and administer prescribed skilled physical therapy treatments for patients recovering from injuries, illnesses, or functional disabilities, helping them achieve their highest level of physical function.
  • Conduct physical strength assessments to assist physicians in evaluating a patient’s functional level, documenting findings to develop or modify treatment plans and establish measurable therapy objectives.
  • Design and implement customized rehabilitation programs aligned with the physician’s Plan of Treatment and the patient’s overall goals, making adjustments as needed for optimal results.
  • Collaborate with physicians and clinical team members to gather additional patient information, contributing to the development, implementation, and revision of therapy treatment plans.
  • Provide Physical Therapy Assistants and Home Health Aide staff with written instructions and care plans, ensuring therapy-related tasks are executed effectively. Supervise and evaluate staff performance while monitoring the accuracy and completion of documentation.
  • Maintain thorough and timely documentation of patient care, observations, interventions, and evaluations. Ensure that interim (verbal) physician orders and physical therapy updates are accurately recorded, submitted for physician signature, and implemented appropriately.
  • Communicate patient progress with physicians, Clinical Managers, staff, patients, and families, submitting evaluations, treatment plans, progress reports, and discharge summaries to supervisors and care management teams.

Required Qualifications & Skills

  • Degree from an accredited Physical Therapy Program (approved by the APTA).
  • At least one year of physical therapy experience (preferred).
  • Current and unrestricted Physical Therapy license.
  • Valid CPR certification.
  • Strong organizational and communication skills to ensure effective patient care and collaboration with the healthcare team.
