About the job
Summary:
This full-time Physical Therapy position will involve providing patient-centered one-on-one care within patients’ homes located in lower Fairfield County. The patients will be geriatric, and the company is CMS certified. The scope of work will include helping patients recover from illnesses, helping to manage chronic conditions, and designing and adjusting care plans to help patients adjust to new limitations that arise.
Responsibilities:
- Make home visits to patients in lower Fairfield County, Connecticut.
- Perform diagnostic tests and measurements.
- Develop and implement appropriate individualized care plans.
- Educate and instruct patients, family members, or other client representatives in rehabilitative care and activities necessary to promote the patient’s health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided on company provided tablet.
Requirements:
- Graduation from an accredited school of Physical Therapy.
- Connecticut State License or eligibility for CT State licensure before the start date.
- Basic Life Support (BLS) certification
- Valid driver’s license.
- Experience with Medicare Part A preferred
