Company Overview
In this role, the successful candidate is responsible for all aspects of medication therapy including review of all inpatient medication orders and outpatient prescriptions dispensing of medication and counseling of patients. Provides guidance and mentors non licensed staff performing approved medication related tasks.
Job Responsibilities

- Enters medication information orders appropriately SIGs Start/Stop Dates etc from written orders D/C s discontinued medications Validates CPOE orders and makes necessary correction to order Screens for possible adverse drug events Documents interventions notes and allergies properly
- Ensures patient safe time effective and appropriate medication therapy Triages and reviews orders for appropriateness of therapy Selects correct drug/dose/dosage form to be labeled Prepares accurate container label appropriate container and affixes the proper auxiliary information
- Participates in the training and supervision of staff and continues to be a mentor and knowledge base for the purpose of enhancing continual staff development Reviews filled orders for accuracy Corrects any errors and educates personnel to the nature of the error and preventative measures
- Complies with policies and procedures related to working in the sterile preparation area; proper hand washing procedure; special handling requirements and procedures for preparation of sterile bio hazardous orders Maintains clean and aseptic workspace; Handles and disposes of sharps bio hazardous materials in a safe manner Reviews chemo orders for appropriate drug dose protocol and regimen Receives co signatures on orders; Maintains proper records for reference on patients for future treatments for Human Resources regarding personnel handling bio hazardous substances
- Maintains accurate patient profiles prescription records including patient s name DOB address and date filled initials or signature etc Screens prescriptions for appropriateness of drug and therapy and possible adverse event potential Dispenses drug with the appropriate auxiliary labeling Offers counseling to patient
- Handles controlled substances in accordance with federal state local regulations and hospital policies and procedures Processes wasted returned and expired drugs Resolves discrepancies and notifies management of those discrepancies
- Conveys pertinent issues to the appropriate personnel to maintain an effective continuum of patient care and makes appropriate documentation for efficient follow up Contacts prescribers directly to resolve medication and therapy issues Notifies Patient care personnel of outstanding issues affecting their patients Informs outstanding issues to Pharmacy personnel working current and out coming shift Interventions ADR s note and other documentation is entered into the computer appropriately
Minimum Qualifications
To qualify you must have a Bachelor’s Degree Pharmacy. Current New York State Pharmacy License. Excellent communication both oral and written organizational detail oriented problem solving and record keeping skills. Basic computer skills. Proficient in the use of the Pyxis system. Knowledge of the special handling requirements and additional policies and procedures for preparation of sterile bio hazardous orders.
