Home Health Occupational Therapist – Responsibilities & Qualifications

Your Role as a Home Health Occupational Therapist:

  • Assess and evaluate patients’ daily living and work-related skills, developing therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic functional tests in patients’ homes or other settings to assist physicians in determining their functional abilities.
  • Collaborate with physicians and healthcare team members to develop and update individualized care plans.
  • Provide therapeutic interventions per physician orders to enhance strength, coordination, range of motion, and function, or teach compensatory strategies as needed.
  • Adapt and refine retraining programs to align with patient goals and rehabilitation guidelines.
  • Work with vocational team members to develop and implement plans tailored to patients’ needs and capabilities.
  • Communicate patient progress and status with physicians, rehabilitation teams, clinical managers, patients, and families.
  • Participate in care coordination and discharge planning, serving as a resource for other healthcare professionals.
  • Develop community reintegration activities to support physical rehabilitation and psychological adjustment, coordinating efforts with the interdisciplinary team.
  • Recommend and/or design adaptive equipment to enhance patients’ living and working environments and overall rehabilitation experience.
  • Train staff and family members on rehabilitation programs, including personal care activities such as dressing, feeding, grooming, and hygiene.
  • Supervise, guide, and evaluate the performance of Occupational Therapy Assistants and Home Health Aides in implementing occupational therapy services.
  • Accurately document patient care, observations, interventions, and evaluations on the day services are provided. Ensure verbal physician orders are correctly recorded and implemented. Submit evaluations, treatment plans, and discharge summaries to supervisors and care management teams.

Required Skills & Qualifications:

  • Degree from an accredited Occupational Therapy program.
  • Minimum of six months of occupational therapy experience (preferred).
  • Home health experience is a plus.
  • Active and unrestricted Occupational Therapy (OT) license.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • Valid driver’s license, reliable transportation, and auto insurance.
