Type: Full-Time, Permanent

Hours: Monday – Friday, 8:30 AM – 5:00 PM

Position Overview: Assess, plan, develop, coordinate, and provide treatment and counseling as a member of the ACT team. Conduct program planning, assessments, consultations, and follow-up on direct services and rehabilitation to assist clients and their families in coping with health-related social problems.

Key Responsibilities:

  • Conduct screening interviews and assessments using various methods.
  • Recommend and modify treatment plans to address medical, psychiatric, and psychosocial needs.
  • Provide ongoing counseling to individuals, groups, or families.
  • Identify service gaps and advocate for needed services.
  • Build strong relationships with patients, families, co-workers, and the community.
  • Provide case management for assigned clients, coordinating and monitoring Individual Treatment Plans.
  • Assist clients in attaining benefits and teach money management skills.
  • Provide crisis intervention and rapid support services.
  • Schedule and coordinate medical and dental treatments.
  • Visit clients during inpatient treatment and plan for discharge.
  • Ensure clients’ basic needs (housing, food, clothing) are met before other interventions.
  • Assist clients with daily living skills and teach transportation skills.
  • Develop and document a psychosocial database.
  • Maintain client progress records according to COMAR and agency requirements.
  • Provide consultation and educational services to community agencies.
  • Participate in interdisciplinary conferences.
  • Provide ongoing assessment and symptom education.
  • Utilize evidence-based practices for co-occurring disorders.
  • Be available for clients at all times and provide on-call crisis intervention.
  • Maintain high fidelity to the ACT Model of Care.

Job Requirements:

  • Must be a licensed Mental Health Professional in Maryland
  • Excellent organizational, problem-solving, verbal, and written communication skills
  • Valid driver’s license and clean driving record
  • Ability to work independently with minimal supervision
