What You’ll Do

As a Licensed Practical Nurse (LPN) in Ambulatory Care, you will provide both direct and indirect patient care under the supervision of a Registered Nurse, Nurse Practitioner, Physician Assistant, or Physician. Functioning as part of an interdisciplinary healthcare team, you may also be required to make home visits.

Key Responsibilities:

  • Deliver patient-centered nursing care, tailored to individual needs.
  • Accurately collect and document relevant patient health data.
  • Assess health data to identify patient concerns and needs.
  • Implement and document specific care interventions.
  • Recognize and address deviations from expected patient outcomes.
  • Follow evidence-based practice standards and approved protocols.
  • Conduct and document Point of Care (POC) testing and ensure compliance with laboratory policies and procedures.
  • Maintain and document the proper storage, handling, and use of testing reagents, supplies, and equipment.
  • Advocate for patients by ensuring their autonomy, dignity, and rights are respected.
  • Provide care in a non-judgmental, inclusive, and culturally sensitive manner.
  • Ensure informed consent is obtained and documented for required procedures.
  • Maintain active licensure, certification, and professional competencies.
  • Provide proof of current BLS certification to the supervisor.
  • Complete all required credentialing, annual training, and competency validations as per department policies.
  • Practice 100% within the LPN scope of practice as defined by New York State regulations.
  • Continuously update knowledge and skills relevant to the role.
  • Perform additional duties as assigned by a supervisor or manager.

Education & Experience:

  • Required: Graduate of an accredited Practical Nursing program.
  • Preferred: Previous experience in a healthcare setting.

Licenses & Certifications:

  • Required: Active New York State LPN license and registration or a valid interim permit.
  • Required: BLS certification (must be obtained within 6 months of hire).

Physical Requirements:

  • Ability to perform heavy work, including:
    • Lifting up to 100 lbs. occasionally, 50 lbs. frequently, and 20 lbs. constantly
    • Occasional climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, stooping
    • Frequent balancing, typing, hearing, repetitive motion, computer use, standing, walking
    • Ability to see color and depth perception
    • Must pass fit testing

Workplace Hazards & Conditions:

  • Occasional exposure to burns, chemicals, electrical hazards, mechanical hazards, and radiation
  • Frequent exposure to bodily fluids and bloodborne pathogens
  • Occasional exposure to noise, dust, fumes, gases, mists, and odors

Travel Requirements:

  • Occasional travel may be required but is not a regular part of the role
