Healing Hands Hospice Care Inc. has been serving NorCal since 2023. We are hiring a Hospice Office Admin LVN with at least 6 months experience. Duties and Responsibilities:

Responsible for Office Administrative tasks and the development and implementation of hospice services, including the coordination with Physicians, interdisciplinary team and patients, manage medications and DME for patients. Attends scheduled IDT and Coordination meetings and work on office administrative tasks.

Responsibilities :

  • Collaborate with physicians, and other team professionals including RN, CHHA, Social Workers, Chaplains to provide care
  • Attend IDG Meetings meetings as scheduled
  • Monitor and QA visit notes on a timely manner inthe EMR systemto be in compliance with HIPPA Regulations , maintain accurate records
  • Notify and update doctor and interdisciplinary teamfor a patient’s condition when deemed necessary
  • Monitor Patient Calendar and coordinate with visit schedules
  • Responsibilities include but no limited to, verify medical insurance, answer phones, data entry, filing, and other basicoffice duties
  • Document basic medical history for intake
  • Produce and distribute correspondence memos, letters, faxand forms
  • Patient care coordinator, such as new/refillmedication, DME orders and referrals

Required education and experience:

  • Current Licensed Vocational Nurse (LVN) license or Licensed Psychiatric Technician (LPT)
  • Basic computer skills including Microsoft word, e-mail and EMR system
  • Ability to coordinate with professionals from diverse backgrounds, working at distinct levels
  • Candidate must have at minimum 6 months of LVN experience
  • Familiar with HIPAA/client confidential and personal rights
  • Current CPR and First Aid certification
  • Valid Driver’s License and vehicle insurance
