Responsibilities:
- Monitor the health and well-being of individuals to ensure high-quality care.
- Administer medications, maintain accurate documentation, and oversee staff responsible for medication administration.
- Provide medical treatments as prescribed by physicians and assist with personal care services.
- Conduct assessments, implement healthcare plans in Individual Service Plans (ISP), and address changes in health conditions.
- Train staff on universal precautions, infection control, and medication administration.
- Serve as a liaison and advocate, coordinating with medical professionals and external agencies to ensure consistent care.
- Educate individuals on self-medication, prescribed treatments, sex education, STD prevention, and smoking cessation.
Qualifications:
- Associate’s Degree in Nursing or a related field, or state certification.
- One year of nursing experience working with the population served.
- Current LPN or LVN license in the state of residence.
- Valid driver’s license, vehicle registration, and auto insurance.
- Certifications as required by state regulations, such as CPR/First Aid, HIV/AIDS certification, Bloodborne Pathogens (BBP), and ACT training.
- Strong communication skills for interacting with staff, individuals served, and families.
