Located in Schenectady, Eddy SeniorCare is an innovative and team-based model of care designed specifically for seniors. Our interdisciplinary team includes physicians, RNs, LPNs, home health aides, physical/occupational/speech therapists, social workers, transportation services, and a day center team. Full-Time Social Worker Role
We are seeking a dedicated Full-Time Social Worker to support participants through phone-based assistance and in-home visits. Responsibilities include:
- Conducting comprehensive psychosocial evaluations
- Coordinating care transitions between hospital, rehab, and home
- Assessing patient and family needs to develop individualized care plans
- Connecting participants to community resources that support independent living
Qualifications:
- Master’s degree in Social Work (MSW) required
- New York State LMSW licensure required
- At least one year of relevant experience in home care, healthcare, or senior services
