Full-Time Social Worker Role

We are seeking a dedicated Full-Time Social Worker to support participants through phone-based assistance and in-home visits. Responsibilities include:

  • Conducting comprehensive psychosocial evaluations
  • Coordinating care transitions between hospital, rehab, and home
  • Assessing patient and family needs to develop individualized care plans
  • Connecting participants to community resources that support independent living

Qualifications:

  • Master’s degree in Social Work (MSW) required
  • New York State LMSW licensure required
  • At least one year of relevant experience in home care, healthcare, or senior services
