Position Overview
The Clinical Nurse Educator and Care Coordinator will play a dual role in ensuring quality care delivery and promoting professional development within our team. This position is ideal for a dynamic Registered Nurse (RN) who excels in patient education, staff training, and care coordination. You will work closely with clients, caregivers, and healthcare professionals to create and implement individualized care plans that uphold our high standards of service.

Key Responsibilities
Clinical Education:
Develop and deliver training programs for caregivers and nursing staff.
Ensure compliance with clinical best practices, company policies, and regulatory requirements.
Act as a mentor and resource for staff, fostering a culture of continuous learning.
Care Coordination:
Conduct initial client assessments and develop customized care plans in collaboration with the client, their families, and the care team.
Oversee and monitor the implementation of care plans to ensure quality and client satisfaction.
Act as the primary point of contact for clients and families, addressing concerns and providing guidance.
Quality Assurance:
Perform regular home visits to evaluate care delivery and caregiver performance.
Maintain accurate and up-to-date clinical documentation.
Lead efforts to improve care standards and client outcomes.
Collaboration:
Coordinate with interdisciplinary teams to ensure seamless care delivery.
Participate in team meetings to share insights and contribute to care planning.
Required Qualifications
Completion of an accredited nursing education degree program
Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
Minimum of 5 years of clinical nursing experience, including 2 years in home care or community health settings with a focus on medically complex situations.
Knowledge of regulatory requirements and best practices in home care.
Current Basic Cardiac Life Support – Health Care Provider (BCLS – HCP)
Proven experience in care planning, patient education, and staff training.
Exceptional communication and interpersonal skills.
Proficiency in electronic medical records (EMR) systems.
Valid driver’s license and reliable transportation.
Responsible to carry equipment, use and operate a laptop computer/tablet and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) up to 30 lbs as a regular part of the job.
Preferred Qualifications
Master Degree in an applicable field or specialty certification in Nursing Education or Case Management preferred.
Experience developing nursing policies and procedures adhering to best practice recommendations in Canada
Experience working in a luxury or high-end service environment.
Bilingual skills are a plus.
