Role Overview

At Oak Street Health, Primary Care Physicians play a vital role in delivering equitable, high-quality, value-based healthcare to Medicare patients in our neighborhood-based primary care centers. We prioritize quality over volume, ensuring physicians have the time and support they need to focus on patient outcomes rather than the number of services provided.

Each physician works at a single medical center, supported by a robust care team, allowing for better patient-provider relationships and an enhanced patient experience. 📅 Schedule: Full-Time, Monday – Friday, 8:00 AM – 5:00 PM

Key Responsibilities

  • Assess, diagnose, and treat patients in our local primary care centers.
  • Oversee and administer comprehensive primary care services.
  • Prescribe and manage pharmaceutical treatments and medications.
  • Maintain accurate patient records using Canopy and Greenway EHR systems.
  • Collaborate with regional and central leadership to achieve healthcare quality goals.
  • Work closely with Practice Managers to lead and support the care team.
  • Perform additional duties as needed.

Required Qualifications

  • M.D. or D.O. degree from an accredited medical school.
  • Board Certified or Board Eligible in Internal Medicine or Family Medicine.
  • Active, unrestricted medical license in the applicable state.
  • Valid DEA license.
  • Authorization to work in the U.S.

Preferred Qualifications

  • Fellowship training in Geriatrics.
  • Experience in geriatric medicine or value-based care models.
  • Prior experience in outpatient primary care settings.
  • Proficiency in bilingual communication (where applicable).
