St. John’s Community Health is an independent, 501(c)(3) nonprofit organization operating a network of community health centers dedicated to serving patients of all ages across South Los Angeles and the Inland Empire.
Our mission is to eliminate health disparities and promote community well-being by delivering high-quality healthcare services tailored to the diverse needs of our communities.
With a network of over 20 Federally Qualified Health Centers and school-based clinics, we provide comprehensive medical, dental, and mental health care to individuals and families throughout Central and South Los Angeles, Compton, and the Inland Empire.
Our primary focus is to bridge healthcare gaps for low-income, uninsured, and underinsured residents by ensuring accessible, culturally competent, and linguistically appropriate services, regardless of a patient’s financial situation.

Education (Preferred):
- Bachelor’s degree or equivalent experience.
Experience (Required):
- Minimum of two years of relevant experience.
- Bilingual skills preferred.
Certifications:
- Community Health Worker Certification (Employees must maintain required certifications and provide verification and recertification upon request).
Essential Duties & Responsibilities
The role includes, but is not limited to, the following responsibilities:
Outreach & Engagement
- Conduct community-based, street, and online outreach to inform individuals and institutions about St. John’s services and partner agency programs.
- Engage individuals and families to connect them with available healthcare and support services.
Health Education
- Provide educational resources to individuals, families, and communities on various health conditions and services, including:
- Diabetes
- Heart disease
- Pulmonary disease
- Hypertension
- HIV/AIDS
- Hepatitis
- Sexually transmitted infections (STIs)
- Substance use disorders
- Mental health conditions
- Social determinants of health (SDOH) and their impact on overall well-being
- Community Health Worker II will provide advanced health education to paraprofessional staff across multiple disciplines.
Screening & Linkage to Care
- Conduct brief screenings to assess service needs for individuals and families.
- Provide direct referrals with warm handoffs to the most culturally relevant services within St. John’s or community organizations.
Assessment & Service Planning
- Evaluate individuals and families using approved assessment tools to determine if SDOH needs are being met.
- Develop personalized service plans that document:
- Identified needs
- Action steps
- Progress towards SDOH goals
- Community Health Worker II will conduct in-depth assessments using clinically intensive tools such as:
- PHQ-9 (Patient Health Questionnaire)
- GAIN-SS (Global Appraisal of Individual Needs – Short Screener)
- Columbia Depression Scale
- Beck Depression and Anxiety Inventories
Case Management, Care Coordination & System Navigation
- Help individuals and families identify culturally competent services aligned with their service plans.
- Provide direct linkage, warm handoffs, and follow-ups for all referrals.
- Facilitate communication between medical and psychosocial service providers to ensure coordinated care and optimal patient outcomes.
Peer Support, Education & Advocacy
- Offer one-on-one support and lay counseling to assist patients in managing their health and service goals.
- Provide assistance such as:
- Appointment reminders
- Transportation support
- Attending medical visits as a cultural mediator
- Guidance on navigating the healthcare and social service systems
Training & Development
- Develop and deliver training programs related to health conditions and social determinants of health (SDOH).
- Provide educational sessions to:
- St. John’s staff
- Community partners
- Medical and social service professionals
