Responsibilities

Purpose Statement

Provide medical or therapeutic care under the supervision of nursing or clinical staff to support patients in their recovery journey.

Essential Responsibilities

  • Foster a safe, structured, and supportive environment for patients under the direction of a charge nurse or supervisor.
  • Ensure patient safety by conducting timely observation rounds, which may include 15-minute checks, within-reach supervision, or one-on-one patient care.
  • Maintain cleanliness and organization in patient rooms to promote a comfortable healing space.
  • Assist patients with mobility, including lifting, transferring, turning, or repositioning as needed.
  • Obtain and accurately document vital signs, height, and weight as assigned.
  • Introduce yourself to patients and visitors promptly and respond to patient needs in a timely, compassionate manner.
  • Engage with patients in therapeutic activities designed to support their treatment goals.
  • Provide calm and non-threatening responses to distressed patients, seeking to understand and address their concerns.
  • Utilize de-escalation techniques to help patients manage emotions and behaviors while assisting in seclusion and restraints when necessary.
  • Monitor and reinforce positive behavioral changes, communicating observations and progress with the treatment team. Immediately report significant changes to the nursing staff.
  • Support patients in applying skills learned from nurses, physicians, and therapists to enhance their physical and mental well-being.
  • Ensure patient dignity, comfort, and privacy while assisting with personal care tasks such as bathing, grooming, dressing, feeding, and changing linens.
  • Coordinate or provide transportation for patients as needed.
  • Maintain reliable attendance and adhere to scheduled work hours.
  • Follow established policies, procedures, and care standards.
  • Clean and sanitize assigned areas, including patient equipment such as beds, IV poles, and wheelchairs.
  • Accurately and promptly document patient care and clinical information in medical records.
  • Collaborate with team members to support patient treatment and rehabilitation.
  • Demonstrate a positive, empathetic, and professional attitude toward patients at all times. Address patient concerns with care and work toward resolution, prioritizing their safety and well-being.
  • Communicate clearly and effectively with patients, families, visitors, and healthcare team members.

Qualifications

  • High school diploma or equivalent required.
