Essential Functions
- Conducts or delegates initial and ongoing patient evaluations within the scope of practice.
- Regularly completes Start of Care, Resumption of Care, Recertification, and Discharge OASIS visits and, as needed, provides routine visits based on patient needs and branch requirements.
- Develops and implements a care plan that incorporates appropriate health maintenance, prevention, and safety education for patients.
- Coordinates available resources to effectively manage care plans and achieve established patient outcomes.
- Periodically reassesses or delegates the reevaluation of patient needs, making adjustments to care plans as necessary.
- Ensures comprehensive patient care through thorough Start of Care assessments.
- Facilitates communication among care team members, physicians, administrative staff, referral sources, and external case managers to maintain continuity of care and update the plan as needed.
- Identifies patient needs and ensures that qualified caregivers are assigned appropriately.
- Participates in special projects and carries out additional duties as assigned.
Required Experience & Skills

- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of two years of clinical home care experience, with at least one year in a clinical management role preferred.
- Current CPR certification.
- Strong verbal and written communication skills.
- Excellent interpersonal skills for effective patient and team interactions.
- Knowledge of state and federal home health agency regulations and compliance standards.
- Familiarity with clinical policies, procedures, and the ability to implement them.
- Understanding of the Patient-Driven Groupings Model (PDGM) structure.
- Proficiency in English (reading, writing, and speaking fluently).
- Commitment to reliable attendance and professional responsibilities.
- Additional duties as assigned.
- Valid driver’s license, auto insurance, and reliable transportation required.
