About the Job

Through advocacy, housing support, and comprehensive services, we aim to enhance the health, well-being, and stability of those we serve.

We are currently seeking a Registered Nurse – Recuperative Care Center to deliver acute and chronic nursing care to patients in our Recuperative Care program. This role involves triaging referrals, working with community hospitals and facilities, and coordinating care plans for patients. This position is located at 2175 California St., Denver, Colorado 80204, and reports to the Respite Nurse Manager.

Credentialing Requirement

To ensure the highest standard of care, the selected candidate must complete an internal credentialing and privileging process before their official start date. The timeline for this process depends on the timely submission of necessary documents. A start date will be scheduled once credentialing is completed.

Key Responsibilities

  • Deliver nursing care to patients at the Stout Street Recuperative Care Center, following community healthcare standards.
  • Collaborate with hospitals, clinics, social service agencies, and internal CCH programs to ensure seamless continuity of care.
  • Triage referrals based on patient acuity, program requirements, and available beds.
  • Provide patient education tailored to individual learning styles, literacy levels, and cultural backgrounds.
  • Offer patient-centered care using an integrated, trauma-informed, harm-reduction approach.
  • Empower patients by delivering relevant health education and information.
  • Work collaboratively with multidisciplinary healthcare teams.
  • Be available to work weekends and holidays as needed.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR), including:
    • Recording clinical services, telephone interactions, and other communications.
    • Managing tasks, PAQs, and scanning to ensure current and complete patient records.
    • Keeping insurance status updated and completing necessary clinical authorization documentation (e.g., CCARs/DACODs).
  • Perform additional duties as assigned.

Qualifications

  • Education: Associate Degree (AD), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN) from an accredited institution.
  • Licensure: Current Registered Nurse (RN) license in Colorado.
  • Certification: Basic Life Support (BLS) certification.
  • Credentialing: Successful completion of Coalition’s privileging and credentialing process for RNs.

Preferred Experience:

  • Ambulatory, public, or community nursing.
  • Mental health, emergency room, urgent care, palliative care, care management, or critical care experience.
