Clinical Coordinator Responsibilities

  • Team Leadership: Provide structured supervision and guidance to a team of Case Managers, fostering a supportive and effective work environment.
  • Clinical Oversight: Implement and uphold best practices such as Housing First, Harm Reduction, Motivational Interviewing, and trauma-informed care to ensure high-quality service delivery.
  • Service Planning: Oversee the creation and execution of individualized service plans, ensuring accurate, timely documentation of client interactions and progress.
  • Client Engagement: Conduct assessments and interviews for prospective clients, encouraging active participation and addressing behavioral challenges with a compassionate approach.
  • Collaboration & Coordination: Partner with service providers and outreach teams to facilitate comprehensive care and support client success and housing stability.

Qualifications & Requirements

  • Education: Must hold a New York State Licensed Masters Social Worker (LMSW) certification.
  • Experience: Minimum of three years working with relevant populations, with supervisory and administrative experience preferred.
  • Clinical Expertise: Deep understanding of Housing First, Harm Reduction, Motivational Interviewing, and behavioral/medical health systems.
  • Skills & Competencies:
    • Strong crisis intervention, risk assessment, and incident management abilities.
    • Effective delegation and leadership skills to inspire and motivate staff.
    • Exceptional verbal and written communication skills, with the ability to engage diverse populations and stakeholders.
    • Technical proficiency in Microsoft Office Suite (Word, Excel, Outlook, Teams); experience with Access-based databases is a plus.
