Key Responsibilities:
- Conduct in-home, problem-focused assessments, including reviewing medical history, performing health screenings, reconciling medications, checking vital signs, and conducting targeted physical examinations.
- Recognize urgent and emergency situations, responding swiftly and appropriately.
- Order, perform, and interpret diagnostic tests, such as lab work, EKGs, and ultrasounds.
- Provide therapeutic interventions, including administering IV fluids and medications (IV, IM, or oral).
- Coordinate diagnostic testing and follow-up care to ensure comprehensive patient management.
- Incorporate patient goals into personalized care plans, emphasizing palliative care principles when appropriate.
- Educate patients on disease processes, medication management, and the importance of compliance.
- Communicate effectively with referring clinicians to ensure seamless transitions of care following visits.
- Support clinical guidelines, care pathways, and quality improvement initiatives to maintain high standards of care.

Requirements:
- Active Physician Assistant (PA) licensure in the state of New York.
- At least two years of experience in relevant healthcare settings, such as Emergency Department, Urgent Care, or Hospital environments.
- Current Basic Life Support (BLS) certification (Advanced Cardiovascular Life Support (ACLS) certification is preferred).
- Physical capability to navigate stairs and provide patient care within home-based settings.
