Keys Responsibilities and Qualification
The Licensed Practical Nurse (LPN) in Home Health is responsible for delivering high-quality patient care and performing skilled technical care within the patient’s home. Working within the scope of practice defined by state nursing laws, the LPN provides care according to the established plan of care and collaborates closely with other members of the healthcare team, without making any alterations to the care plan. Additionally, the LPN educates the patient’s family on proper patient care and how to maintain a safe and healthy environment. They are also responsible for observing, documenting, and reporting the patient’s response to treatments and any changes in condition to the supervising nurse or physician on a daily basis.
To qualify for this role, candidates must hold a current LPN license in the state of practice, possess a valid driver’s license, have reliable access to a vehicle or public transportation, and maintain current CPR certification.
