Job Overview:

We are hiring a Registered Nurse Assessor for the New York Independent Assessor Program (NYIAP). In this role, you’ll conduct health assessments to determine eligibility for long-term community-based services for Medicaid recipients (ages 19+). Assessments are conducted via telehealth or in-person at the consumer’s home, with a flexible schedule based on consumer-driven appointments.

Key Responsibilities:

  • Conduct health assessments (telehealth or in-home) for Medicaid-eligible individuals.
  • Review and summarize medical documentation to determine care needs and eligibility.
  • Enter assessment data into the UAS-NY online system and document all appointments.
  • Collaborate with the clinical team to ensure accurate and high-quality assessments.
  • Stay updated on changes in regulations and assessment guidelines.
  • Travel within your assigned region to meet with consumers in their homes.

Requirements:

  • Active RN license in New York
  • Experience in Home care a minimum of 2 years.
  • Strong organizational and communication skills, with proficiency in Microsoft Word and Outlook.
  • Ability to conduct Zoom assessments and navigate web-based systems.
  • Valid New York State Driver’s License and the ability to travel for in-home visits.
  • Reliable high-speed internet
