About the job

We are seeking a dedicated and skilled Certified Wound Care Nurse to join our healthcare team. The ideal candidate will have a strong background in wound and ostomy care and has worked with post-acute, home health, or community-health patients. This role is primarily in the office, Monday – Friday 9 AM -5 PM, but may require some travel in NYC for wound care consultations.

Responsibilities

  • Collaborate with the home health team to review potential patients for program.
  • Assess eligibility for admission including a comprehensive assessment of wounds.
  • Input patient data and formulate a wound/ostomy treatment plan.
  • Participate in the development, evaluation, and revision of all wound and ostomy management policies, ensuring evidence-based practices are implemented.
  • Provide family education/counseling regarding wound care, risk factors associated with pressure injury, infection prevention, treatment measures, etc.
  • Participate in departmental/interdisciplinary conferences, meetings, and quality improvement initiatives relating to wound, ostomy, and continence issues.
  • Monitor progress and response to treatments, making adjustments as needed.
  • Maintain accurate medical records and documentation related to wound care.
  • Participate in caseload reviews and chart audits where appropriate and develop interventions to correct identified deficiencies.
  • Stay updated on the latest wound care techniques and best practices.

Experience

  • Current NYS RN License
  • Certification in Wound Care or Wound Ostomy Care required
  • Proficiency in Microsoft Office and EMR Systems (Epic preferred)
  • 2+ years of experience: CHHA, Post-Acute , community health, and/or med-surg
  • Previous experience in PI is preferred
