Summary:

We are seeking a Wound Care Registered Nurse to join our team and provide specialized care in wound, ostomy, and continence management. This position involves conducting health assessments, developing care plans, educating patients and staff, and collaborating with the healthcare team to provide optimal patient outcomes. The ideal candidate will be experienced in wound care, ostomy, and continence care, with certification as a Wound Care Nurse. You will play a vital role in improving patient care by providing comprehensive assessments, interventions, and education, while ensuring high standards of care delivery.

Key Responsibilities:

Conduct health assessments focused on wound, ostomy, and continence care, performing a physical assessment, and developing individualized care plans.

Recommend appropriate medications and treatments based on patient needs and diagnosis.

Develop, revise, and evaluate standards of care, including selecting supplies, technology, and equipment.

Assess and document patient response to wound care, ostomy, continence, fistula, and complex wounds treatment.

Evaluate the quality of care provided by clinical staff on the unit and collaborate with healthcare teams for optimal care delivery.

Provide patient and family education on prevention and management of complications related to wounds, ostomies, and continence issues.

Develop, implement, and evaluate educational programs for staff, including new employee orientation and continuing education.

Provide assessments for patients at risk for developing pressure injuries and initiate preventative measures.

Escalate challenges and communicate ethical concerns or conflict resolution issues as needed.

Collaborate with the Assistant Director of Nursing for Wound Care to recommend and initiate best practices.

Participate in Quality Improvement (QI) activities, including incidence, prevalence studies, and nursing quality improvements.

Deliver culturally competent education and care in line with patient and family beliefs, ensuring respect for diverse backgrounds.

Maintain confidentiality and security of patient data in accordance with patient rights policies.

Assist patients in accessing community resources that can support their care.

Manage rented wound care equipment, such as specialty beds and negative pressure devices.

Skills & Qualifications:

Required Experience:

  • 2 years of med/surg nursing experience with wound and colostomy care.

Required Education:

  • Valid New York State RN License.
  • AHA BLS (Basic Life Support) certification.
  • Required Certifications:
  • Current certification in one of the following:
  • Certified Wound Ostomy Continence Nurse (CWOCN) by the Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB).
  • Certified Wound Ostomy Nurse (CWON) by the WOCNCB.
  • Certified Wound Care Nurse (CWCN) by the WOCNCB.
  • Certified Wound Specialist (CWS) by the American Board of Wound Management (ABWM).
  • Wound Care Certified (WCC) by the National Alliance of Wound Care and Ostomy (NAWCO).

Skills:

Expertise in wound care and home health nursing.

Strong communication skills and ability to educate patients, families, and staff.

Ability to work independently while managing patient care and documentation.

Organizational skills to handle up to 15-20 consults per day.

Must be Wound Care Certified.

Schedule Notes:

  • This position requires the nurse to work 70% in-office and travel to patients’ homes in Manhattan, Brooklyn, Bronx, and Queens for wound care consults.
  • The nurse will manage 15-20 consults daily, providing wound care services to patients across multiple locations.
